Prior to your interview with a ReVIDA Care Coordinator, please bring the following items with you for the interview.
- Please bring all forms of identification – a valid Drivers License, State ID, or Military ID, Social Security Card and a Photo ID if possible.
- Prepare a list of any prescribed medications.
- If available, please bring your current Insurance card.
- Please arrive 15 minutes prior to your appointment.
